WHAT IS YOUR NEW PATIENT PROCEDURE?

Contact me directly via phone or text at 502-795-1986 and we will decide together when the best time would be to set up an initial evaluation. After the initial evaluation we will again decide together what will be the best personalized treatment for you!

WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?

HomeLife Therapy is a cash based (or "out-of-network") therapy practice, so no insurance is accepted. Don't let this idea scare you off just yet- rather take a moment to learn why we believe this is the best option for you!

By eliminating insurance companies from the equation, we are able to focus on high quality, personalized therapy. You will be treated one-on-one by a skilled professional for a minimum of 30 minutes, but will typically be 60 minute treatment sessions. Evaluations may take a little longer, depending on the complexity of the situation and treatment performed during the initial evaluation. No more technicians doing most of your session or treating multiple patients at a time. Working out-of-network allows more freedom to treat your problems the way they should be treated, without the constraints of the insurance company.

It's also important that you know your insurance company MAY COVER out of network Physical therapy. If this is the case, at your request, we will provide you with a “super-bill” after your final session to submit to your insurance company for reimbursement.

DO I NEED A DOCTOR’S REFERRAL?

Kentucky is a direct access state for Physical Therapy, so no referral is needed from your physician. You will be evaluated and treated one-on-one without ever having to go see your physician. If not in our scope of practice, we will make referrals to appropriate healthcare providers to make sure your needs are met.

Your insurance company MAY COVER out-of-network Therapy so, at your request, we will provide you with a “super-bill” after your final session to submit to your insurance company for reimbursement. You should let your insurance company know that you have already paid in full for services received. We are not affiliated with Medicare, so we ask that you do not submit claims to Medicare or the possible secondary insurances with which you may be affiliated.

CAN I SUBMIT TREATMENTS TO MY INSURANCE COMPANY FOR RE-IMBURSEMENT?
